A combination of a couple of things including die deterioration and just plain wear on the coin itself.

Thanks form the info.If you can notice in the scan,the area of the mm is also sunken in.Looks like a small d inside a larger d.The coin is worn,however you can stii see some hair details and some of the ear.Only some heads visible on the reverse.Lines smooth. Old die caused this?
