At first I thought this was a die crack but looking closer I am questioning whether it is a scrap piece of plating retained. 1994D
Any thoughts

Looks like a die break with a die crack tail on it. Usually debrie is found on the outside quarter of the coin. But anything is possible.

Since it is a 1994, it might be possible that it has a gas bubble under the die crack that is forming something like a lamination. Maybe???
