A good article is in CW about "waffle canceled" coins escaping from the mint.
The mint has lately started defacing all non-standard (read: "error") coins. No canceled coins are supposed to make it out but apparently several found their way to eBay.
The auction had 1 of each denom (how conveeeeenient) from the cent to dollar. There's no way this was an accident, IMO. Just like the quarter/Sacagawea mule.
BTW the equipment used for the in-house mutilation of defective coins was built by a Dutch company. Hup Holland Hup!
