Obverse: A die scratch in the loop of the 9 of the date. N-S die scratches through LIBERTY with a short heavier scratch from the top of the B in LIBERTY. Numerous die scratches below and to the tight of the ST of TRUST.

Reverse: A die crack from the O of ONE to the rim. A heavier die scratch runs between the lower T and E of UNITED. A long die scratch through the A of states, and continues south through the motto. Reverse is LDS.
