Obverse: A large die crack runs through the forehead horizontally to the top of the ear. Old die damage can be seen as die dots and lines to the east of the Y of LIBERTY.
Reverse: A heavy die dot shows inside the lower center of the N of CENT. A small but heavy die scratch connects the center of the letters N and I of UNITED. Reverse is LDS.
